How do I format this? - how to write a cover letter as a physical therapist aide

I have a final version is expected today. I have a lot to do, but a part, and I have no idea how to do this because I have never written a proposal. I thought I found my answer last night, but led to more than writing a business proposal. I'll post what my teachers.
